If you are a smoker or have any medical conditions which may affect your life expectancy, we could get you an even higher income.  If this applies to you please print of the Client Information Form which includes a Medical Evidence Questionnaire.  Once you have completed and signed your form, please return to Bank of Scotland Annuity Service
